MacArthur Campus Media Reservation Request Form

To ensure availability of DVDs and videos at time of need, faculty may provide dates or syllabi to Reserve staff who will then remove requested items from general circulation two weeks in advance of show date. Holds and recalls from the library catalog cannot be made for DVDs and Videos.

Upon completion of this form, you can expect an email response from Jupiter staff confirming your request. If you receive no reply within two business days, please call the library at 6-8530.
