Digital Services Request Forms

Use the appropriate online forms below to submit requests that you may have regarding services from the Digital Library.

Please allow 24- 48 hrs for processing. NO rush services will be provided. In order to best accommodate your request, we ask that you submit your request 1-2 weeks in advance. Before submitting a form, please make sure that you have obtained permission from your supervisor or the appropriate designee. All our forms should only be used for submitting requests that relate with academic or scholarly purposes. If you have any other questions, contact the Digital Library Offices at lydig@fau.edu

• Internal Release Form : This is an example of an internal release form that you can use to get permissions from the appropriate person before using their materials for online/other uses

• Scanning Request Form: [Internal Library-Use Only] Use this online form to submit any scanning requests for textual, photographic, sound or video items

• Digital Project Partnership Form: Use this online form to request collaboration with the Digital Library for creating or developing new online digital collections

• Podcast Recording Request Form: [Internal Library-Use Only] Use this online form to submit requests to record podcast of an event, seminar, presentation etc.

• Request Form for Item Inclusion Into An Existing Collection: Use this form to request the addition of item(s) into an existing digital collection.

• Suggest a new Digital Collection: Use this request form to propose the addition of new collection(s).
